These Urgent Suspected Cancer (USC) Referral forms represent the West Midlands Cancer Alliance's agreed regional standard for USC referrals, developed in line with national guidance and in collaboration with regional partners.

While the clinical content has been ratified at WMCA level, individual ICBs may apply local branding or make minor administrative adjustments prior to approval and deployment within their own referral systems.
